Clay Beresford (Hayden Christensen) suffers from a heart condition that requires a heart transplant. When he receives a call informing him that a donor has been found, he believes that his life has been saved. But, during surgery, Clay experiences a phenomenon called "anesthetic awareness," leaving him awake but paralyzed throughout the operation. To escape the pain, Clay has an out of body experience, and discovers that his new wife, Sam (Jessica Alba), and his trusted friend, Dr. Jack Harper (Terrence Howard) are involved in a plot to take his life.
